Tender Touch Home Health Care Inc., accept referrals from the physician, social worker, other health team member, third party payer, client, family friend or another home health care company. The services should be Medicare reimbursable and has the need for a minimum of one (1) skilled home care service provided on an intermittent, part time or full time basis. The client should be under the care of a Licensed Physician and the place of residence are within the geographic area served by the agency, and a plan of care is required.
Services are provided in a coordinated team approach to therapeutic, rehabilitative and supportive intermittent health care services of highly trained, Licensed Professional and Experienced Staff.
